You don't do an IPA app without Xcode, or some very tricky things.  Much easier to just provide a .zip for users to unzip and manually put into place, like WD7 Experience Enhance Mod Alpha v0.11 does, or make a .deb file that handles backup/restore of files on upgrade, install, and removal using a shell script.  The first is on the users end if they mess up, the latter is all automated, but requires some very basic coding knowledge and time on your end ;)

You don't need to distribute the whole .ipa file, just the bits you modified (graphics I'm guessing).

I thought the official youtube app that wasn't installed on 3.1.3 by default was not compatible with iOS 3, at any point. I thought it was released around iOS 5 :o

If you mean a .ipa file of youtube, and not just a modified .app folder of the stock youtube app, then yes. Otherwise, no.

If all you have is a modified official youtube app from /Applications on the device, you should not be distributing that first of all, and second, it won't install as a .ipa file.  Third, you should only distribute the modified files, not the whole folder. So pretty much just images ;)
