Below is a step by step guide on how I activated MMS and Video messaging on my 2G while running 3.1.3.
I decided to post this because for the past two weeks i have been scouring the internet for help. After many failed attempts this is what worked for me and maybe it'll answer many questions.
MY biggest problem that i had was that i was able to send pictures on 3.1.2, but could not receive due to a "Size or Resolution Limit", i still cannot get MMS to work on my phone under the 3.1.2fw.
Here is what i used:
Windows 7 Laptop
Iphone 2G Model A1203
Itunes 10
redsn0w 0.9.4
Files Needed:
iPhone1,1_3.1.2_7D11_Restore.ipsw------- I'll explain why

IOS boot loaders 39 and 46
First - I forced an upgrade to 3.1.3 through iTunes.
This caused my Jailbreak and Unlock to go away(Expected)
Second - Opened up redsn0w selected the 3.1.2 ipsw firmware.
For some reason the 3.1.3 would not read. I downloaded multiple files from different sites and they all did the same thing. I kept getting the file is not recognized error. Remember you will still have 3.1.3 but for some reason using 3.1.2 will jailbreak it and unlock it too. Don't ask me why, but it just did
Third - Completed the Jailbreak and Unlock through redsn0w (There is plenty of guides on that, so no need to add that information.
Fourth - Open Cydia and do all the updates
Fifth - Add the fallowing repositories to Cydia
Smart-Mobile.com
xsellize Repo
http://apt.thebigboss.org/repofiles/cydia/ ---- This one should be installed by default, if not just add it
Sixth - Power down phone after the repo's are installed and restart Cydia. I did this because for some reason the repo listing did not update until after the restart.
Seventh - Search and Install packages in this order.

Up to this point is where i was stuck because the Camera button would not show and it did not matter how many times i would restart the phone the button would not show up. I had and was able to manually change all the MMS settings under the Network Data, but none of them would do anything.
A this point if the camera button does not show up try and re-install the "Activate MMS (2G) 3.1.3" a couple of times, re-starting the phone after each time and try to send message and see if the camera button shows up. It took me 4 times for it to show up.
Now that i had the button i still could not receive pictures because of that annoying "size or resolution error".
After looking through multiple forums, i assume that the error might be the different phone models, which would explain my some people can activate it easily and others have problems.
What i used to fix the problem was a fix in the XSellize repo called T-Mobile iOS4 MMS Fix and choose the one that says U.S.A only
After this is done walla, it now should work. If MMS settings dont work, uninstall file and install a different one from a different repo since there are multiple ones available. Some of them say ATT/tmobile carrier bundle or Carrier Bundle, but they all do the same thing, but might have different codes, since T-Mobile have 3 different APN's and a couple Proxies
On the pictures below you can see that the phone camera now has a "Camera/Video" switch witch i never saw before
